Maple Tree Health provides patient navigation and care coordination services for specialty medical practices. As a HIPAA-compliant Business Associate, we work on behalf of medical practices to support patients with follow-up care, treatment adherence, and ongoing communication.
Our services help ensure patients stay on track with prescribed care plans, medications, and recommended follow-up, with support from their provider’s care team between visits.
Communications are conducted on behalf of medical practices and limited to existing patients who have provided consent.
